Adult Day Health (ADH) services are Medicaid Waiver or Fee-for-Service services provided to participants in order to assist them in developing, maintaining and improving their skills in daily living. A Service Supervisor will work with each participant to develop and implement a plan to assist the individual in working towards his or her goals. Some areas of assistance include:

Applying for Adult Day Health Services
Applying for Adult Day Health Services Participants and their families should begin the application process by contacting their case manager. Participants should also work with their circle of support to gather the following documents. If families are able to pay out-of-pocket for ADH services, they should contact The Arc to inquire about the availability and fees for the needed/desired services.
- Current tuberculin (TB) skin test or chest x-ray
- Current physical examination
- Copy of Social Security Card
- Copy of birth certificate
- Current photo identification (State ID, passport, etc.)
- Documentation of diagnosis of an intellectual and/or developmental
disability by a psychologist/psychiatrist - Notice of Privacy Practices form
Adult Day Health Center Locations
Adult Day Health Services take place both at The Arc centers and in the community. All of the centers operate Monday through Friday from 7:30am to 3:30pm. Centers are closed on weekends and holidays. For a tour of the facilities and the program, please contact the individual center.
